What ages do you care for?
We care for infants, children and adolescents starting at ages 6 months through 18 years.
What types of visits do you offer?
- Acute sick visits
- Sports physicals
- Camp, daycare, and kindergarten assessments. We can even manage most college admission physicals.
- Compliance-based and paperwork visits
Where do visits take place and about how long do they take?
Visits Locations:
- At your home
- At school or community locations (when appropriate)
How long do the visits take?
Most visits last 30–45 minutes, allowing time for assessment, education, and questions—without feeling rushed.
The best part is that there are no waiting rooms and no long lines.
What happens when I book online?
Booking online is fast, easy, and secure:
- Select Your Visit Type – Acute care, sports physical, or school/daycare assessment
- Pick Date & Time – Same-day or next-day visits available based on location
- Register your child by entering Your Child’s Info – Name, age, medical history, and visit details
- Confirm address of visit
- Receive Confirmation – Check your Email and text with appointment details and instructions
- Provider Prepares – We review your child’s info to ensure a focused, efficient visit
- Enjoy Your Visit – We arrive on time, bring supplies, complete the visit, and provide any forms
Booking online makes pediatric care convenient, stress-free, and easy for busy families.
Do you offer same-day or weekend appointments?
Yes. Same-day and next-day visits are often available.
Weekend visits are often based on availability and have to be booked through the office.
Do you accept insurance?
We only bill the standard plans of NC Medicaid(Healthy Blue, Carolina Complete, UHC, AmeriHealth) We do not bill private insurance. Families may pay by cash, credit/debit card, Cashapp or HSA/FSA funds and may submit receipts to their insurance if desired.
Pricing
We operate on a Pay for Service model, which allows longer visits and transparent pricing.
- Acute Care Visit: start at $95 (One Add’l Test Included)
- Sports Physical: Start at $45 (Vision Included)
- Camp/ College/ School / Daycare / Kindergarten Assessment: start at $55(Vision Incl’)
*There are additional fees for rapid testing and/ or services*
Pricing includes the visit and same-day completion of required forms (If Applicable).
Do you offer bundles?
Yes. Visit bundles are available for families who want to plan ahead, save money, and stay prepared for unexpected needs.
What if my child needs urgent or emergency care?
If your child needs a higher level of care, we will guide you to the appropriate next step and help coordinate care when needed.
Is there a convenience or travel fee?
The convenience travel fee starts at $35 and is based on distance traveled.
- Travel within our primary service area is included (within 20 miles radius)
- Areas Included: All of Forsyth County, Bermuda Run, Advance, Eastern Lexington, Colfax, Oak Ridge, Western Guildford County)
- Visits outside this area include an additional fee and schedule availability, which will be discussed before booking.
This helps cover travel time and allows us to continue providing mobile, in-home care
When to Call Purposeful Pediatrics:
Fever 100.4 or greater for more than 24 hrs
Mild Signs of dehydration
Sudden rash with fever
You feel something isn’t right
When to Call 911 Immediately:
Trouble breathing/Blue Lips
Seizures
Severe lethargy
Significant Signs of Dehydration (No Urination in 8 hrs or more)
Respiratory
Call us if:
Using rescue inhaler more than every 4 hours
Nighttime coughing
Wheezing not improving after treatment
Constipation
Call us if:
No bowel movement for more than 4-5 days
Painful bowel movements
Blood in stool
Severe abdominal pain
Cold & Viral Illnesses
Call us if:
Fever lasts more than 2 days
Symptoms worsen instead of improve after 5–7 days
Severe ear pain, sinus pain, or persistent cough
Child is not drinking fluids well
Child has any fever (100.4°F or greater)
Ear Pain
Call the office if your child has:
Ear pain lasting more than 24–48 hours
Fever over 102°F
Drainage (pus or blood) from the ear
Pain not improving with Tylenol or Motrin
Repeated ear infections
Rashes
Call us if:
Skin is red, warm, and painful
Yellow crusting present (possible infection)
Severe itching not controlled with home care
- Spreading or changing in appearance over 1-2 days
Influenza
Call us if:
High fever lasting more than 2 days
Severe body aches or headache
Vomiting that prevents drinking
Symptoms improve then suddenly worsen

Pink Eye
Call us if:
Thick yellow or green discharge
Eye swelling
Child complains of eye pain
Symptoms last more than 24 Hours
Sore Throat
Call us if:
Sore throat with fever
Swollen glands
White patches on tonsils
Headache and stomach pain with no cough
- Muffled Voice/Difficulty Swallowing
Urinary Discomfort
Call us if :
Pain or burning with urination
Frequent or urgent need to urinate
Accidents in a previously potty-trained child
Foul-smelling or cloudy urine
Fever with urinary symptoms
Lower belly or back pain
Vomiting & Diarrhea
Call us if:
Vomiting lasts more than 24 hours
Diarrhea lasts more than 3 days
Blood in vomit or stool
Signs of dehydration
